Excessive growth of the lining of the uterus. This can often be better handled through hormonal therapy using natural or synthetic progesterone. Progesterone causes the excess lining of the uterus to shed itself.

Uterine prolapse. Women can also do Kegel exercises (see prenatal exercise chapter) or use pessaries to hold the organs in place. Sometimes small amounts of estrogen may be used vaginally to strengthen tissues. If symptoms are severe, the best course of action may be a surgical repair and/or reconstruction of the uterus, bladder, rectum and their supporting structures. Sometimes, a severely prolapsed uterus may have to be removed.

Stress incontinence or leaking of urine when you cough or sneeze. There is no evidence that hysterectomy will help this problem, in fact it may make it worse.

Chronic PID can benefit from complete bed rest and prompt and adequate treatment of flare-ups. Even for acute PID with infection of the whole abdominal cavity, intravenous antibiotics and drainage is safer than hysterectomy. All this as well as commentary on other forms of treatment such as acupuncture are contained in the excellent booklet put out by the PID Society in Vancouver. Be sure to read the chapter on PID before making your final decision.

Severe Endometriosis. The number of hysterectomies done for endometriosis has increased steadily over the years and only recently seems to be levelling off. Unfortunately too many women recover from hysterectomy to discover that their pain is still there. Endometriosis can have many appearances and occur in many places in the pelvis. Therefore, hysterectomy does not solve the problem, and may add many new problems. As Dr. Herbert Goldfarb says in is book, THE NO HYSTERECTOMY OPTION (John Wiley and Sons 1990), "It is extremely rare person in this day and age who should ever need a hysterectomy for endometriosis, no matter how severe the condition."

Before making any final decision, please consult the two chapters on endometriosis in this book, especially the work of Dr. David Redwine. The Endometriosis Association has the latest information about surgery and drugs as well as information about what all your alternatives are. You can also buy time by taking medications for a short time while you do your reading and research.

Chronic pelvic pain not due to any of the above conditions. In this case, there is little proof that a hysterectomy would help.

Dr. R.C. Reiter studied chronic pelvic pain in women who had normal laparoscopies. At leat 50 percent of these women had sources for their pain that were missed, like bowel or bladder problems. Of course if your doctor didn't know how to look for endometriosis, he may have missed unusual appearances of endometriosis on the bowel or bladder.

Fifty percent of the women were found to have their pain related to stress, depression and especially common was the history of sexual abuse or trauma. In the latter case, "The last thing a woman needs," says Reiter, "is to have another assault on her sexual organs."

Uterine fibroids are the cause of about one third of hysterectomies. Fibroids are non-cancerous growths on or in the uterus. They are harmless in themselves, but sometimes cause bleeding, pain or pressure symptoms.

Moderate vaginal bleeding. This can be due to many causes including fibroids and hormonal imbalance. Other solutions should definitely be explored first including hormonal medications.
